Who’s Eligible: Class Members include U.S. residents who purchased Metagenics products that were labeled “Medical Food” since Nov. 9, 2011. Metagenics products include UltraMeal Plus, UltraMeal Plus 360, UltraGlycemX, and UltraClear.

Proof of Purchase: Class Members can submit claims for up to five products without proof of purchase. Class Members submitting claims for more than five products need to provide proof of purchase, such as receipts.
